Are you someone with drive, commitment, and a passion for resolving housing barriers? If so, we have an exciting opportunity for you! We are looking for a Housing Coordinator to join our Care and Health department at WDH.

What you’ll be doing:

In this role, as a Housing Coordinator, you'll will work as part of a multi-disciplinary team to provide housing related information to patients and staff, ensuring a secure and sustainable hospital discharge for inpatients at Pinderfields, Dewsbury, and Pontefract hospitals.

Your responsibilities will include assessing patients' housing situations, identifying barriers, and providing support to facilitate the effective discharge of patients from hospital back into the community. You'll also be proactive in identifying patients requiring housing support and prioritise early housing support for individuals with underlying health issues to maximise their opportunity to successfully sustain accommodation.

What we’re looking for:
To join our Wellbeing department, we’re looking for someone with:

  • a qualification at level 3 of the Regulated Qualifications Framework;
  • self-motivation and the ability to respond effectively when under pressure to meet deadlines in a performance orientated culture;
  • the ability to manage a challenging caseload and multiple interventions at any one time;
  • experience of delivering intensive support to vulnerable individuals and families experiencing social and health inequalities; and
  • the ability to demonstrate an understanding of equal opportunities in service delivery.
