We are looking for someone to join our Care and Health team to provide support and advice to tenants who would benefit from interventions and support to enable them to overcome barriers and make positive changes to their lives.
What you’ll be doing
In this role you will take responsibility for assessing the needs of individuals referred to the service and create and manage a support plan aimed at improving their overall wellbeing.
You will provide holistic one-to-one support for tenants addressing areas such as mental health, financial stress, housing concerns etc. You will help them access appropriate services where necessary with of view of helping them sustain their tenancy with WDH.

This post also requires applicants to have a valid UK driving licence and daily access to a vehicle in order to travel throughout the district and to other locations as required by the business.
