Wellbeing Case Workers and Mental Health Navigators referral form

The Wellbeing Case Workers role is to work with individuals and families within the Wakefield district to tackle inequalities in health.

The Mental Health Navigators are employed by South West Yorkshire Partnership Foundation Trust NHS and seconded to work for WDH. The Navigators role is to provide holistic mental health assessments, signpost and pathway to relevant services and provide short-term therapeutic work when a need is identified.

All tenants referred must consent to the referral and must be willing to work in partnership with the Wellbeing Caseworkers and Mental Health Navigators to achieve positive outcomes.

The service will support tenants to better understand the actions they can take to manage their health and wellbeing by making and maintaining positive changes to their lifestyle.

Tenants will be supported to address the underlying issues which prevent them from adopting healthy lifestyles and the emphasis is on supporting them to develop their skills.
