Health Inequality Case Workers

Our vision is to create confident communities, however we do recognise that confident communities are the product of confident individuals. Therefore we are committed to changing the way we work with individuals to enable them to gain the confidence to lead a healthy life within the communities they want to live.

Working with NHS Wakefield we have appointed five Health Inequality Case Workers based in multidisciplinary teams whose role it is to offer intensive support to individuals and families who would benefit from the development of their health and wellbeing skills.

Part of their role is to establish an effective working relationship with statutory and voluntary sector services and the local community infrastructure in order to enable clients to maximise benefits from resources available.

Whilst their main focus is on health issues they also address Financial Inclusion and work closely with our Debt Team to ensure that clients have access to mainstream financial products such as bank accounts, low cost loans and grants.

They also work closely with our Community Employment Advisors who can assist clients with Training/Employment opportunities as well as helping with CVs and job applications.
